TOTP is computed from the shared secret and current time. A clock that is noticeably out of sync can produce the wrong code even when the secret is correct.
SHA-1, six digits, and a 30-second period are common defaults, but an otpauth:// URI can specify SHA-256 / SHA-512, eight digits, or another period.
